Alguns estudos in vitro demonstraram que materiais restauradores como o amálgama
e o ionômero de vidro apresentam características ou componentes que proporcionam
atividade antibacteriana (TYAS, 1991; MORRIR et al., 1998; MOUNT, 1999; MJÖR;
TOFFENETTI, 2000; WANG; LIU, 2000; HAYACIBARA et al., 2003; BRAMBILLA et
al., 2005) e, consequentemente, conseguem inibir cárie recorrente em laboratório (LOBO et
al., 2005; ITOTA et al., 2005). No entanto, um estudo realizado por Mjor & Gordan (2002)
demonstrou que cerca de metade das restaurações de amálgama e ionômero de vidro que
foram substituídas num período de dois ou três anos, foram-no devido ao diagnóstico de cárie
secundária. Os resultados controversos entre os experimentos laboratoriais e clínicos não
permitem afirmar se a incidência de cárie secundária pode ser significantemente reduzida
pela liberação de flúor destes materiais (WIEGAND et al., 2007).Possivelmente, essa
discrepância de resultados pode ser justificada pelo fato de que em restaurações recém-
confeccionadas há liberação do flúor em maior intensidade e que este padrão anticariogênico
diminui com o passar do tempo. Ou seja, à medida que a restauração envelhece há uma
menor liberação de flúor (RANDALL; WILSON, 1999).
No entanto, os modelos in vitro não oferecem uma série de fatores envolvidos no
desenvolvimento da cárie, como por exemplo: a ação da saliva e de substâncias provenientes
do metabolismo bacteriano (RANDALL; WILSON, 1999). Adicionalmente, embora não se
deva desprezar a utilidade de modelos in vitro, geralmente estes modelos não são adequados
como preceptores da eficácia clínica dos agentes testados (ERICKSON; GLASSPOOLE,
1995). Em se tratando de materiais fluoretados, isto se torna ainda mais preocupante já que
implica em uma relação dose-resposta exagerada quando comparada àquela obtida em
estudos in vivo. É importante ressaltar também, que em sistemas in vitro, em que se utilizam
tampões ou géis ácidos, o efeito na concentração de flúor nos fluidos orais da ação de
limpeza da saliva bem como, o da presença de outras fontes de fluoretos que ocorrem in vivo,
não são considerados. Além do mais, a liberação de flúor pode até ocorrer da mesma forma
que acontece na boca, no entanto, a desmineralização é grandemente acelerada em modelos
estáticos (ERICKSON; GLASSPOOLE, 1995). Quando modelos in vitro microbiológicos
são usados, o estágio de remineralização e o controle do pH são difíceis de serem obtidos, a
não ser que sejam utilizados fermentadores ou “bocas artificiais” (TANG et al., 2003). Até
onde vai o conhecimento dos pesquisadores envolvidos neste estudo, tais dispositivos não
estão disponíveis em nosso país na atualidade.
Neste contexto, os estudos in situ apresentam significante relevância, devido a razões
éticas e às vantagens de um melhor controle experimental das variáveis, além de uma melhor
